Job Summary/Basic Functions
The program coordinator is responsible for working with primary care practices for successful implementation of evidence-based interventions, population health strategies, and quality improvement activities to establish organized approaches to breast, cervical, and colorectal cancer screenings within clinical settings. This position is responsible for managing and cultivating relationships, assessing clinical workflow and processes, using practice facilitation, and coaching to drive quality improvements related to increased cancer screenings. Works under limited supervision. Finally, the program coordinator will assist in the implementation of the early detection and prevention programs for cancer. Experience with diverse clinic EHRs such as EPIC, Athena, and ECW, and overlay dashboards such as Azara is required.
Minimum Qualifications
Required Education: Bachelor's
Required Field of Study: Health or Health care related discipline such as Public Health, Policy, Administration, Health Communications, Quality Improvement or related
Number of years required to perform the job: 5
Type of experience required to perform the job: relevant work experience; healthcare related discipline
Substitution: Master's Degree with 2 years of professional level experience may substitute for required experience. Must have experience in quality improvement preferably in a clinic setting.
